First Visit
Your first visit with us is a consultation with the doctor. He will discuss your diagnosis and explain your procedure and its benefits with you. If your dentist gave you a referral and x-ray please bring them with you to this appointment. It is also very important that you bring a picture ID and your insurance information with you. Bringing us your insurance information will benefit you by allowing us to verify your benefits for you, and then apply them towards your surgical treatment plan so that your out of pocket expense is kept to a minimum.
We also have patient registration forms that can be filled out prior to your first visit. We encourage you to go ahead and fill out the patient registration forms prior to your visit in order to expedite the check in process. Also, please bring a list of any current medications the patient is taking.
It is important that every patient that is a minor (under the age of 18), be accompanied by a parent or legal guardian. If the person accompanying the patient is a legal guardian, we will require appropriate legal paperwork and a picture ID. If a patient is a minor and is not accompanied by a parent or legal guardian we will have to re-schedule the appointment.

at the time of consultation and at the time of surgery.
IMPORTANT: It is Imperative that you alert your attending surgeon if you have any changes to your health and medications prior to surgery and following your initial consultation.
